Appointments
APPOINTMENT CANCELATION POLICY
There is a $75 fee if your appointment is cancelled within 24 hours. No shows will be charged a $75 fee, no exceptions.

Insurance
Call the office in advance at (631) 557-3043 to confirm that your subtype of insurance is accepted and check with your insurance plan to determine whether or not you need a referral or preauthorization for Mohs surgery (codes 17311, 17312, 17313, 17314, 17315).
Generally the following plans are accepted:
AARP, Aetna, Blue Cross Blue Shield, Cigna, Emblem GHI/HIP, Empire Plan, Fidelis, Healthfirst, United Healthcare, Oxford Freedom, 1199.
